Transitioning of Medical Care Policy

We keep a record of the health care services provided to you. You may ask to examine and/or request a copy of your records. You may also ask to correct that record.
  1. A copy of this policy may be given to any patient requesting a transfer of care to another facility.  It is also intended for the release of medical records to a specialist or other provider involved in the care of the requesting patient. This policy will also be posted on our website.  The process begins with the patient announcing they plan to leave the practice or request release of their medical records for whatever reason..
  2. The patient will sign a release of Medical Information Request with the information of where the records are to be sent.  The information of where the records are to be sent is the responsibility of the requesting patient.  We will not do any searches for any addresses or names of any new physicians.  Our office will send the most recent three medical visits and associated laboratory reports and any additional pertinent medical information such as special tests or reports after being reviewed by the doctor or doctor’s assistant.  There may be a charge associated for this service.
  3. The requested records will be sent as soon as conveniently possible depending on the workload at our office.  Any associated charges for this service needs to be paid in advance prior to us sending the records.  We will choose the method of delivery such as US mail, scanning, or fax.  If there is a medical urgency of any reason or type, the doctor will be happy to speak directly with the new physician and provide whatever immediate medical information that is necessary.  It is the new physician’s responsibility to initiate the telephone call.  Certainly, by law, the signed Medical Information Release Request must be completed and received at our office.  We will accept this release by fax if necessary.
  4. We will be happy to provide refills of current medications for 60 days after notice of leaving our care.  We will not provide new medications or evaluate new problems without an office visit which we will be happy to schedule.
